Yes. To be eligible for a grant, applicants must provide a W9 with their tax identification number.
Applicants must upload ALL required documents to the online application. Follow the instructions on the Attachments page of the online application. High-speed internet access will streamline the application process, but it is not required. The maximum size for all attachments from an applicant combined is 25 MB.
If you forgot to submit an attachment, please e-mail it to firstname.lastname@example.org. We ask that you include your organization name in the e-mail subject line.
Not really, but it is a 2-step process. To submit your application, you will first PREVIEW it. After you review it you select “Submit your USSSF Grant Application Form”.
You will know that you have successfully submitted your form when you see a webpage that thanks you for your submission. You will also receive an email confirmation later. If you did NOT receive an email confirmation, contact email@example.com to make sure your application was received.

Once approved, grantees must submit all interim and final reports. The Final Grant Report is due no later than July 31. Contact email@example.com for report templates.
