Yes. To be eligible for a grant, applicants must provide a W9 with their tax identification number.
Applicants must upload ALL required documents to the online application. Follow the instructions on the Attachments page of the online application. High-speed internet access will streamline the application process, but it is not required. The maximum size for all attachments from an applicant combined is 25 MB.
If you forgot to submit an attachment, please e-mail it to sandra.mahoney@usaartisticswimmingfoundation.org. We ask that you include your organization name in the e-mail subject line.
You will know that you have successfully submitted your form when you see a webpage that thanks you for your submission. You will also receive an email confirmation later. If you did NOT receive an email confirmation, contact sandra.mahoney@usaartisticswimmingfoundation.org to make sure your application was received.
Before the submission deadline, yes. We do recommend printing your draft application and carefully proofreading your responses before submitting an application. Please contact us at sandra.mahoney@usaartisticswimmingfoundation.org if you have questions or concerns regarding your application.
Artistic Swimming Foundation uses CaptainForm system to accept applications and applications via the Internet. CaptainForm’s Amazon servers are highly secured and backed up daily. Once approved, grantees must submit all interim and final reports. The Final Grant Report is due no later than July 31. Contact sandra.mahoney@usaartisticswimmingfoundation.org for report templates.
